Academic Regulations Committee (ARC) Reinstatement Form – If you have previously enrolled at USF and had a USF GPA less than 2.00 at the end of your last term of enrollment, you must file an ARC petition in conjunction with your application for admission. The petition must be accompanied by the ARC Advising Record for ARI and ARII.
Application for Undergraduate Admission - If you are unable to submit an application online, download and complete this form. Mail to the Office of Undergraduate Admissions to the address provided. We strongly recommend you submit your application online in order to expedite review of your admissibility to USF.
Application Update Form - If you wish to update your term of entry, change your major or change your home campus.
Conduct Statement - If you answered “yes” to either of the conduct questions on the Undergraduate Application for Admission. Include any supporting documentation.
Freshman Admissions Deposit - If you are a freshman applicant who has been offered admission to USF, submit a non-refundable $200.00 enrollment deposit by May 1 to reserve your space at USF.
International Financial Support Document - If you are an international student on an F-1 visa, please provide proof of financial support
Residency Affidavit - If you claim to be a Florida resident for tuition purposes, verify your information with this form. Students under 24 typically must provide parental information rather than their own.
Third Party Authorization - If you are living abroad and wish to give permission to another party to receive information on your behalf.
