  1. RISK MANAGEMENT INSURANCE RESPONSIBILITIES

    The University has insurance coverage under the State of Florida's self-insurance fund. Chapter 284 of the Florida Statutes provides the legal authority for the Florida Insurance Trust Fund and the Florida Casualty Insurance Risk Management Trust Fund. The Insurance Coordinator is responsible for the University's insurance needs, processing the claims and premium payments. University departments are responsible for reporting to Environmental Health and Safety injury and/or property damages.

      This section describes the types of insurance coverage, self-insured and commercial, that the University has and how to initiate various claims.

    2. Responsibilities of Risk Management, Insurance Coordinator

      1. Coordinating all insurance coverage for the University, including commercial and self-insured. The exception is the Medical Professional and Premises Liability Insurances and the Workers' Compensation Insurance.
      2. Serving as the initial information contact for anyone injured on University property, other than employees. Employees injured while on duty are covered by Workers' Compensation Insurance.
      3. Referring claims to the Division of Risk Management, in Tallahassee, for processing. Claimant will be sent a copy.
      4. Providing the State of Florida Comprehensive General Liability Insurance Certificate to vendors and lessors, as required by specific agreements.

    3. Responsibilities of Individual Departments

      1. Filing notices of injury or property damage with Risk Management, Insurance Coordinator. Automobile Accident Reports and General Liability Loss Reports are available online and in Environmental Health and Safety, CRS 104.
      2. Notifying Risk Management, Insurance Coordinator of a new building or location where USF owned contents are housed and how buildings are to be covered by completing a Facilities Insurance Data Form.
      3. Report to Risk Management, Insurance Coordinator the location and the value of State-owned inventories every January 1st.

  3. AUTOMOBILE LIABILITY

    1. Fleet Automobile Liability Insurance Certificate of Coverage.

      All vehicles owned by the State have liability but not comprehensive or collision insurance coverage. Contact Risk Management, Insurance Coordinator for a copy of the Fleet Automobile Liability Insurance Certificate of Coverage for additional information if needed.

    2. Brochures to be Retained in State Vehicles

      Brochures are maintained in all State vehicles that describe what to do in the case of an automobile accident. Vehicles are automatically insured when received by the Univeristy. Contact Risk Management, Insurance Coordinator for a copy of the brochure that describes Fleet Automobile Liability Insurance information, this includes how to report a claim, who is covered, things to remember in case of an accident, and safety tips if needed.

    3. Automobile Accident Report

      Automobile Accident Report forms are to be completed for any accident involving a University vehicle. Once completed, the report is to be forwarded to Risk Management, Insurance Coordinator, CRS 104, along with copies of Police Reports, repair estimates, and any other pertinent information. The Report and all support documentation is submitted by the Insurance Coordinator to the Division of Risk Management in Tallahassee. The Report is then reviewed by an Insurance Specialist to determine liability. Contact Risk Management, Insurance Coordinator for a copy of an Accident Report form if needed.

      Any accident resulting in a serious injury or death should be reported immediately, by telephone, to Risk Management, Insurance Coordinator. If an accident occurs after normal business hours, notify University Police.

  4. BOILER AND MACHINERY

    Premium payments are coordinated by Risk Management, Insurance Coordinator. Claims made for equipment damage are filed directly by a representative from the Division of Physical Plant with the appropriate insurance carrier.

  5. CRIME INSURANCE

    Provides coverage for employee dishonesty, excess employee dishonesty and theft: robbery, burglary, sneak theft, etc.

  6. ELECTRONIC DATA PROCESSING (EDP)

    Provides coverage for electronic data processing systems, electronic communications equipment, and air conditioning equipment used exclusively in the operation of electronic processing systems and electronic communications equipment. Also, "Electronic media" is covered. This includes converted data and similar property under the University's care, custody, or control.

  7. FEDERAL CIVIL RIGHTS

    This insurance provides Federal Civil Rights Liability Coverage provided pursuant to Chapter 284, Part II, Section 768.28, Florida Statutes.

  8. FIRE AND EXTENDED COVERAGE

    University-owned buildings and their contents are insured for natural causes only. NOTE: There is no coverage for theft or loss.

    Fire and Extended Coverage type claims are filed via memorandum with Risk Management, Insurance Coordinator. If a claim is filed for a lightning strike, a notarized statement must be submitted along with the Claim form. Refer to Exhibit 14-1 for a sample memorandum regarding a Fire and Extended Coverage type claim. Fire and Extended Coverage type claims are sent by Risk Management, Insurance Coordinator to the Division of Risk Management in Tallahassee for processing.

    Repairs are made at the expense of the University. After the repairs are completed, a copy of a Purchase Order and Invoice are forwarded to Risk Management, Insurance Coordinator who, in turn, forwards the documentation to the Division of Risk Management for processing. An Insurance Specialist then determines if the claim is covered and the amount of loss. The University will be reimbursed.

    In the case of a loss involving a serious injury or death, contact Risk Management, Insurance Coordinator immediately, by telephone. If such a loss occurs after normal business hours, notify University Police.

  9. GENERAL LIABILITY

    General Liability Insurance is coverage for injuries or property damage for which the University is found to be negligent. Contact Risk Management, Insurance Coordinator for a copy of the General Liability Certificate of Coverage for additional information if needed.

    General Liability Loss Reports are to be completed for any incident for which an individual wants to claim personal injury or property loss. The General Liability Loss Report, along with copies of the Police Report, doctor or hospital charges, and any other pertinent information, is forwarded to Risk Management, Insurance Coordinator. In turn, the Insurance Coordinator forwards the General Liability Loss Report and any support documentation to the Division of Risk Management in Tallahassee. Claimant will receive a copy of the letter sent to the Division of Risk Management. The Report is reviewed by an Insurance Specialist to determine liability. Refer to Exhibit 14-2 for a copy of a General Liability Loss Report form.

    Any accident resulting in a serious injury or death should be reported immediately, by telephone, to Risk Management, Insurance Coordinator. If an accident occurs after normal business hours, notify University Police.

  10. HEAD START

    This insurance provides coverage for excess medical expense for the University's Head Start program.

  11. INLAND MARINE

    This provides coverage for fine arts and/or antiques and can also cover consolidated equipment.

  12. INTERCOLLEGIATE ATHLETICS

    Provides medical coverage for student athletes who participate in intercollegiate athletics.

  13. LAW ENFORCEMENT ACCIDENTAL DEATH

    This coverage provides accidental death and dismemberment.

  14. MARINE

    This insurance provides hull (physical damage) and Protection and Indemnity coverage for research vessels.

  15. SUMMER CAMP

    Summer Camp provides coverage for participants in day camps or resident camps sponsored by the University. Contact Risk Management, Insurance Coordinator for additional information. To secure coverage for participants in day camps or resident camps sponsored by the University, the following procedures are required.

    1. Coverage must be requested prior to the event. (The department representative sends a memorandum to Risk Management, Insurance Coordinator requesting coverage for the event, the details of the event, i.e. date of camp, the number of participants and what kind of coverage is needed, such as accident or health coverage.
    2. Risk Management, Insurance Coordinator then contacts the appropriate insurance vendor to contract the services.
