FAQ's - What to Do If Injured At USF

Q:  What are the immediate steps to follow if someone is injured at USF?

A:  Determine if medical care is needed. Notify your supervisor. If medical treatment is required, go to one of the medical facilities. To report an accident, supervisors or department designees must call 974-5775 from
7:00 a.m. to 5:00 p.m. when injured employee seeks medical care. WC Specialist at USF will contact CorVel Corp. and verify insurance information with the medical provider. If an injury that requires medical care occurs on a weekend or a weekday before 8:00 a.m. or after 5:00 p.m., or while on travel status, ask the medical facility to contact CorVel Corp. at 1-866-786-3351 to obtain approval.

Supervisor or department designees should complete (3 forms), First Report of Injury or Illness, Accident Investigation Report and Medical Release Statement.

All forms should be delivered to CRS 104 within one working day or a fine may be assessed.

Q:  What if someone’s injury appears life threatening?

A:  First, carefully assess the extent of the injury.  Immediately call 911.  The 911 operator will contact University Police and Tampa Fire and Rescue.  Tampa Fire and Rescue will dispatch a team to stabilize the victim, render aid, and/or transport to the nearest Emergency Room.  Do not move the victim.  For more University Police information please click on their website: http://upolice.up.usf.edu/.

Q:  Where are the nearest medical facilities?

A:  For students and student employees, the closest facility is Student Health Care, building SHS 100, located next to the Book Store.  The phone numbers is (813) 974-2331; their campus website is:  http://shsweb.shs.usf.edu/.

For employees follow one of the links below for an approved Medical Facility:

For visitors to the campus, at own expense treatment can be found at one of the three locations mentioned above or the injured party may seek care from their own personal physician. 

Q:  Do employees also have the option of going to their personal physician? 

A:  NO!  Medical treatment for work-related injuries are only covered by physicians from the Managed Care Arrangement (CORVEL) Preferred Provider Organization (PPO) network.  To avoid delay in treatment and a possible penalty it is highly recommended to use network physicians from the time of injury.  Otherwise, the employee may then be responsible for payment.  Personal health insurance will not pay for work-related injuries.

Q:  Can student employees injured on the job seek medical treatment at the Student Health Center?

A:  Yes, by all means.  The injured student(s) need to proceed immediately to the Student Health Center.  On occasion students may be referred to one of the nearby medical facilities for more extensive care or follow-up treatment.

Q:  What if the student is not an “employee” and is injured on campus?

A:  The Student Health Center is designed to treat all USF students.  If the student is not on campus pay roll, he or she can still be treated by using their personal insurance or whatever arrangement can be made to seek treatment.  No student will ever be denied treatment.

Q:  Are student employees eligible for Workers’ Compensation?

A:  Yes, if a student is injured while working in an FWSP, OPS or GTA position, they are eligible for Workers’ Compensation as long as they are receiving wages from a departmental account. 

Q:  Are volunteers eligible for Workers' Compensation?

A:  Volunteers and some “volunteer” students working in a laboratory environment, etc are also covered if they meet the USF volunteer hiring criteria.  Coverage will not be compensable if the work is for the student’s own academia gain.  These students must use their own personal insurance in case of an industrial injury.

Q:  What is available for students, not employed by USF, who are hurt in an accident that occurs “after hours” or on the weekend?  What is available?

A:  Students can still be treated by using their personal insurance (HMO/PPO) at designated locations.  The Student Health Center is closed on the weekend except for telephone consultation with a physician at (813) 974-2331.  Our advice is to consult with the physician on call to get expert advice as well as discuss recommended treatment or course of action.

Q:  What about an employee who is hurt in an accident that occurs “after hours” or on the weekend?  What can be done?

A:  Accidents occurring after hours or on the weekend should call the Managed Care Arrangement (CORVEL) directly at 1-800-929-0107 to obtain approval for treatment. The three medical facilities mentioned above in the second question are open during the day on the weekends.  If an accident occurs in the evening, University Community Hospital Emergency Room is the closest available facility.

Q:  Is it important to report an injury?

A:  Absolutely!  All injuries should be reported.  Supervisors/instructors or simply a “designee” must promptly report the accident to Workers’ Compensation.  Call (813) 974-5775 if the accident occurs between 8:00 am – 5:00 pm.

Q:  Are there forms that need to be filled out for Workers’ Compensation?

A:  Yes.  Supervisors/department designees must complete a First Report of Injury or Illness form within 24 hours of the accident.  Forms are available in Adobe Acrobat and downloadable at the link below:

Q:  Is there an Emergency Room on campus?

A:  No.  The nearest Emergency Room is at the University Community Hospital, located at 3100 E. Fletcher Ave, phone (813) 971-6000.  Otherwise, If in doubt or unsure what to do, call 911 for assistance from University Police.

Q:  I cannot find my question among this list?
