When the event is not an emergency... Notify your supervisor, who will complete a First Report of Injury or Illness (FRII). If your injury requires medical treatment, go to one of the medical facilities.
If you require medical treatment, ask the medical facility to contact your employer at 974-5775 or CorVel Corp. Workers’ Comp. at 1-866-786-3351 for any problems. You should not pay for any expenses due to the work-related injury.
Stay in close contact with your supervisor, updating him/her on your work status determined by the doctor. Give all medical statements, ability to work slips, and physical therapy slips to your supervisor. Without proper documentation, you will be unable to receive your WC benefits. Contact the WC Specialist at USF for any information or help at any time. And, most importantly...follow your doctor's orders and get well soon!
