Summary of Benefits

Employees are entitled to receive medical treatment for a work-related injury or illness from a physician authorized by CorVel Corp.. All diagnostic tests and referrals to specialists must be approved in advance, including physical therapy and chiropractic treatment.

Employees should be reminded not to use their personal health insurance cards at any medical facility or pharmacy when the injury/illness is work related, otherwise it may take a lot of time to rectify the billing.

CorVel Corp. Workers' Compensation Treatment Access Card should be presented upon the first visit to the approved medical provider as well as when filling prescriptions. In some cases, the treating facility/pharmacy may call the WC office at USF or CorVel Corp. to verify the claim.

Employees are entitled to receive medicine and medical supplies prescribed by an authorized physician at no cost. These prescriptions can be filled free of charge at any Walgreen's when the CorVel Corp. Workers' Compensation Treatment Access Card is presented by the injured employee. Employees should not use their personal health insurance prescription cards. If the employee is unable to present the card, or go to a pharmacy other than Walgreen's, the pharmacy should call CorVel Corp. at 1-866-786-3351 for authorization. If there is a problem with the authorization, ask the pharmacy to call 974-5775.

Light Duty

USF has implemented a light-duty return-to-work program and encourages physicians to return employees to work as soon as it is determined that employees are able to work with or without restrictions. It is important that the supervisor and employee work together in order to coordinate the light duty work. A light-duty program can be developed with the help of the worker's comp. case manager and the worker's comp. physician.

Disability Leave

USPS, A&P, or Faculty, may be granted up to 40 hours of disability leave (DL). This benefit is paid leave and is not deducted from sick or annual leave balances. It may be used intermittently for medical appointments and/or when the authorized physician has certified in writing that the employee is unable to work. All use of DL must be approved by the supervisor and must be supported by written documentation from the doctor. FWSP, courtesy appointments, OPS and volunteers are not eligible for disability leave, however, FWSP and OPS may be eligible for impairment benefits.

Impairment Benefits (Wage Loss/Salary Benefits)

Once disability leave is exhausted, the injured employee may be eligible to receive impairment benefits. If the physician certifies that the employee is unable to return to work after seven (7) calendar days, the employee will be paid 66 2/3 % of his/her Average Weekly Wage up to a maximum of $683.00 a week, the maximum compensation rate (MCR) for the 2006 year, and may be supplemented by the use of accrued leave. The MCR is subject to change each year and is set by the Florida legislature. An employee may also be eligible to receive partial impairment benefits if he or she is returned to part-time work.

Attendant Care

When an injured employee is not able to take care of daily living activities, home health services may be provided when prescribed by an authorized WC doctor. Attendant care must be approved and arranged through CorVel Corp..
