How To File A Complaint - USF DEO Complaint Process & Forms

Students, staff, and faculty are strongly encouraged to report allegations of discrimination, harassment, and/or retaliation covered under the University’s Diversity and Equal Opportunity Policy (“Policy”) #0-007. Delaying the immediate reporting of an allegation often hinder a proper investigation and can also unnecessarily subject the Complainant to continued discrimination, harassment, and/or retaliation.
Expectations for the DEO Complaint Process

DEO has developed the documents below to advise Complainants, Respondents, and Supervisors of what to expect from this office when an allegation(s) has been filed.
Incident Report Form

This form is to be completed by any employee or student who would like to notify DEO that an incident has occurred that is covered under the Policy.

Upon DEO’s receipt of the Incident Report Form, DEO will contact the appropriate individuals for further information. DEO will evaluate the information provided and will determine if further processing is needed.

This form may be mailed or hand delivered to DEO.

Intake Form

The Initial Intake Form must be filed with DEO within 120 calendar days of the alleged discrimination, harassment, and/or retaliation. The Associate Vice President may waive this time period based on the circumstances of each case.
