Be sure to pay close attention to the
aid year on the form that you are
completing. If you submit a form for the wrong aid year, your requirement
will not be satisfied, and you will be asked to submit the form for the
correct aid year. Information gathered on financial aid forms may change
year to year due to changes mandated by the federal government. Required forms for students are generally found in OASIS. Go to the Financial Aid menu in OASIS and click on "My Requirements, Bookstore Authorizations, and Deferments". If you have any outstanding requirements, click on that link and be sure to select the form for the correct aid year. Following is a list of forms that you can only submit if they are posted as requirements for you in OASIS:
-A--B-Budget Change of Housing Plan (HSCHG) -D- Dependent Support Verification Form (DSVF) -F-Federal Drug Eligibility Worksheet (DRGWKS) in OASIS if required. Federal Stafford Loan Entrance Interview in OASIS if required. Federal
Stafford Loan Exit Interview Federal Stafford Loan Lender Request in OASIS if required. Federal Verification Worksheet - Parent Federal Verification Worksheet for Students (FVWI) in OASIS if required. -G- Graduate Admissions Status (GADM) in OASIS if required. -I- -L- Loan Adjustment Request. Use this form if you wish to cancel, reduce, reinstate or increase your loan(s). IMPORTANT: You cannot submit a cancellation request if your loan funds have already disbursed. You must meet with a financial aid counselor if you want to return all or part of your disbursed loan funds. -P- Parent Low Income Verification (PLIV) Parent Worksheet #3 Verification (PWK3) Pell Graduate/Degree Prior (PELGRD) in OASIS if required. -S- Selective Service Verification (SSER) Social Security Benefits Verification (SSB) Student Low Income Verification (SLIV) in OASIS if required.
