Special Accommodations Form

The Office of Orientation and the University of South Florida are dedicated to providing the same great Orientation experience for all students and their guests. In compliance with the Americans with Disabilities Act (ADA), we provide special accommodations to students and guests who require such to participate in the session.

You have indicated that you or your guest will be needing special accommodations during your Orientation session. Please complete the form below to submit this request. We will contact you within 5 business days to discuss your needs in more detail.

Any meals served during Orientation will include a vegetarian and vegan option. Food items are clearly labeled with ingredients to assist you in identifying items to which you may be allergic. Additionally, Dining Services staff are available in all dining venues to answer any additional questions regarding potential allergens in a particular food item.

All requests should be made at least 10 business days prior your scheduled Orientation session date.
