Change Beneficiary Information
It is good practice to check your
beneficiary information for accuracy on occasion and to keep this
information up to date. If you have one of the following benefits and
need to change or update beneficiary information, please follow the
instructions below.
State of Florida Group Life Insurance: To receive a statement of
current beneficiary information, please call the Prudential Record
Keeping Division at 1-800-778-3827, select option 2, and then option 2
again. When the Prudential representative answers, ask for a copy of
your beneficiary information. They will verify the mailing address, and
you should receive the statement in 5 to 7 days. If the information is
incorrect or needs to be updated, complete a
Change of Beneficiary form and send to Prudential directly or submit
to your local Human Resources Office.
The Gabor Agency Life Insurance Products: Contact the Gabor
Agency at 1-850-894-9611 or 1-800-330-6115 to request current
beneficiary information and to update or change beneficiary information.
Florida Retirement System Pension Plan: Check your beneficiary
information on the Internet at the
Florida Division of
Retirement Online Services or contact the Division at 1-850-488-5207
to request current beneficiary information. If the information is
incorrect or needs to be updated, submit a new
FRS Pension Beneficiary Designation Form to your Human Resources
Office.
Florida Retirement System Investment Plan: Contact CitiStreet at
1-866-446-9377 to request current beneficiary information. If
information is incorrect or needs to be updated, mail a new
FRS Investment Beneficiary Designation Form directly to CitiStreet
or submit to your Human Resource Office.
Optional Retirement Program: Contact your provider company to
request current beneficiary information and to update or change
beneficiary information.
Tax-sheltered Annuity Program: Contact your provider company to
request current beneficiary information and to update or change
beneficiary information.
