Completing the Appointment Status Form (ASF)

To get to the Appointment Status Form, start at the employee's record that is to be changed.  
Administer Workforce>Administer Workforce (U.S.)>Use>Job Data
Here is the employee's Job Record #0.  This information will display above the appropriate fields on the Appointment Status pages.
 
APPOINTMENT STATUS 1
The data from the GEMS record is above the data entry box for each field.  If there is no data above the box, that information is not populated in the GEMS record.

Click on an item for information about that item.

The Effective Date is the only required field.  This is the day the action begins.  For a termination, the Effective Date is the day after the last day worked.

Complete only the fields in which information is changing.

Use the drop down arrows to select the appropriate values.

 
APPOINTMENT STATUS 2

Click on an item for information about that item.

 
APPOINTMENT Compensation
Click on an item for information about that item.

 

To change the pay, enter the appropriate Comp Rate Code in the Change To box and the rate for that code.  To add additional pay components, click on the yellow "+" button to the right of the pay component.
