The Request for Non-Recurring Compensation form replaces both the Appointment Form (Human Resources) and the Certification Form (Payroll).
Purpose

One-time, lump-sum payment upon the completion of work
performed.
Intermittent payments upon the completion of
different portions of work.
Payments

All non-recurring payments are made from OPS appropriations.
Work must be completed before payment will be
issued, advanced payments are not processed.
Taxed a flat 28% unless the employee has
claimed "EXEMPT" on their W-4 Form.
Approvals - Extra
Compensation (Dual Compensation)

Can only be made on a
fiscal year basis. If portions of work continue into the following fiscal year,
a new Dual Compensation Form/Extra State Compensation Form must be completed.
New Hire Paperwork for new employees who are not in
GEMS

All forms and documents
should be completed and submitted to the appropriate Appointment Representative
no later than the 3rd day of initial employment.

Foreign Nationals may not claim
"EXEMPT" on this form (if their country has a treaty with the United
States then the
appropriate 8233 forms for exemption
